Dental Services

 

Patients prepare documents and contact the Medical Records Unit on the 1st floor of the H.R.H. Princess Maha Chakri Sirindhorn Building, to register during the specified hours.

  • Office hours: Monday – Friday 07:30 a.m. – 3:00 p.m. (except Wednesday afternoon from 1:00 PM – 4:30 PM)
  • After office hours: Monday – Friday 4:30 p.m. – 7.00 p.m.
  • Weekends: Saturday – Sunday 7:30 a.m. – 3:00 p.m. (Closed every Sunday, 3rd week of the month)

Prepare Documents:

  • Bring a national identification card or government-issued card with photo and 13-digit identification number.
  • For children without a national identification card, use a birth certificate, house registration, student ID, or government-issued card with photo and 13-digit identification number.
  • For foreigners, use a passport or other government-issued document with an identification number.

1. Fill in the “Medical Records Application Form” ( For online registration *Thai Citizen Only*: Inform the hospital staff at document checking/service system selection desk that you have already completed the registration process online)

2. Submission of Documents: Deliver the “Medical Records Application Form” to the document checking/service system selection desk, along with a national identification card or any government-issued card.

3.Document Review: Staff at the Medical Records Unit will review the submitted documents and give advice on service systems to patients.

4.Selection of Service Systems: Patients select service systems and submit their document at counter no. 5.

5.Patient Identification Card Number Issuance: Upon document submission, staff will record your information, take a photo, and issue a patient identification number.

6.Verification of Information: Your medical record files will be assembled and checked for accuracy. You will be asked to sign to confirm the accuracy.

7.Receipt of Patient Identification Card: Medical Records Staff will provide you with a patient identification card and send you to contact the screening nurse.

8.Screening Process: Proceed to the screening desk, where a nurse will screen for congenital diseases and drug allergies.

9.Check Coverage: Determine your coverage for dental services at Direct reimbursement counter. 

10. Treatment Initiation: Once the screening process is complete, contact to receive treatment at the clinic.

Procedures for Requesting Services for Former Patients:

Patients should proceed to receive treatment at the appointed clinic as scheduled.

For Self-support service system (Dental treatments by dentists) and Pediatric Dentistry Clinic:

– Patients should directly contact the clinic.

 

  • Place patient identification card: Bring the card to either counter no. 3 or 4 at the Medical Records Unit. Place the hospital card in the designated box for old patients, located on the 1st floor of the H.R.H. Princess Maha Chakri Sirindhorn Building. (In case the patient does not bring their own dental patient identification card, they should write their full name instead.)

  • Medical Record Check: Medical Records staff will check the status of the patient’s medical records, which may take approximately 20 minutes.

  • Screening Process: Staff will check symptoms or signs of illness and cross-reference the patient’s medical records. After that, they will transfer patients for treatment at various clinics as required.

  • Check Coverage: Determine your coverage for dental services

  • Treatment Initiation: Patients will be transferred to the appropriate clinic for treatment. They will then need to contact the clinic to receive further treatment.
